Using this form you can claim under your professional indemnity cover in respect of legal proceedings against you resulting from acts of error or omission, good samaritan acts, libel/slander and public liability.

Using this form you can claim for legal expenses incurred by you in pursuit of legal proceedings in respect of your contract of employment or a breach or employment related legislation providing that the dispute arises from your profession.

Using this form you can claim for legal expenses incurred in representing you in connection with activities within the scope and expentent of your profession in respect of: A GPhC Fitness to Practice or Appeals Committee hearing; an NHS tribunal, Primary Care organisation enquiry, MHRA or CQC Investigation; a coroner's inquest or fatal injury enquiry where you are deemed an interested party in the inquest proceedings.

Using this form you can claim for legal expenses incurred arising from your profession in: defense or criminal prosecutions, appeals against Improvement Notices under the Health and Safety at Work Act or the Food Safety Act.

Exclusions: Arising from HMRC Investigations; allegations of offeces against the person or dishonesty; allegations of speeding or driving under the influence or alcohol or drugs or allegations of non endorsable road traffic offenses.
